Exposure of the Hip by Limited Posterior Trochanteric Osteotomy and Transgluteal Dissection for Total Hip Arthroplasty

Abstract

|
We used combined approach of limited posterior trochanteric osteocomy and transgluteal dissection for total hip arthroplasty. Initial purpose was to decrease the possibility of postoperative posterior dislocation of the hip and acquire
beeter
exposure of the acetabulum, both of which are occasionally problematic in posterolateral approach. 26 hip in 25 patients of total hip arthroplasty were evaluated.
This modified posterolateral approach provided exellent exposure of acetabulum and femur enabling to shorten the operation time. Another advantage was the posterior stabllity provided by the strong posterior curtain, by which early
rehabilitation was
possible without the risk of posterior dislocation of the hip. But this approach might no+ be eligible for bsteoporotic patients because of frequent fragment breakage.
